Role of flap fixation in reducing seroma formation in patients undergoing mrm in a tertiary care hospital of Pakistan

Abstract
Objective: To evaluate the efficacy of flap anchoring in preventing seroma formation in patients of breast carcinoma managed by modified radical mastectomy.
Methodology: This study was carried out at department of surgery, JPMC, Karachi from Ist November 2020 to Ist November 2021. A total of 95 patients were included in the study. Non-probability consecutive sampling was used. All patients underwent modified radical mastectomy and the flap fixation using absorbable sutures to reduce the dead space. Data were analysed by SPSS 21.
Results: The study included 95 patients with mean age of 59.76±4.34 years (range 30-65). 42(44.2%) patients had <23 BMI and 53(55.8%) more than 23 BMI. 48(50.5%) patients had disease for less than 6 months and 47(49.8%) more than 6 months. 67(70.5%) patients were married, 12(12.6%) were unmarried and remaining were divorced, widows or separated. Seroma formation developed in 22(23.2%) patients and 73(76.8%) were free of this complication.
Conclusion: Seroma formation continues to be the most common breast surgery complication along with other complications that increase patient discomfort, increases chances of infection may lead to flap necrosis. Flap fixation is a valuable operation following modified radical mastectomy as it reduces dead space and thus reduces likelihood of seroma.
