Effectiveness of metformin in recurrent miscarriages in a woman with polycystic ovarian syndrome

Abstract
Objective: To evaluate the efficacy of metformin in treatment of recurrent miscarriage in women with polycystic ovary syndrome versus a placebo.
Methodology: This randomized controlled trial was carried out at Department of Gynecology and Obstetrics, Imran Idrees Teaching Hospital, Sialkot Medical College, Sialkot from 1st January to 30th June 2023. Women were divided in two groups. One group continued oral metformin 1000-2000 mg daily up to 24th weeks with folic acid 5mg daily in first trimester act as cases and other group stopped metformin after the confirmation of pregnancy acted as control. Patients were followed to detect any pregnancy loss in two groups. Data were analyzed in SPSS 25.
Results: In metformin group, there were 26 (86.7%) cases and in control group there were 13 (43.3%) cases. Mean age of cases in metformin group was 27.33±5.09 years and in control group 30.87±6.36 years. Mean gestational age in metformin group was 26.77±3.42 weeks and in control group it was 26.77±3.42. Effectiveness was observed in 4 (13.3%) cases in metformin group and 17 (56.7%) cases in control group the treatment was not effective. Effectiveness was significantly high in metformin group as compared to control group (p<0.05).
Conclusion: For non-obese women with PCOS, metformin is an ovulation induction drug having benefits over clomiphene and other standard therapies for anovulatory infertility.
