Role of the CHA2DS2VASc Score as a short-term clinical predictor in chronic kidney disease patients undergoing regular hemodialysis

Abstract
Objective: To evaluate the role of the CHA2DS2 VASc score as a short term clinical predictor in chronic kidney disease (CKD) patients undergoing regular haemodialysis.
Methodology: This retrospective cohort study analysed CKD patients undergoing regular haemodialysis at a tertiary hospital in Medan, Indonesia. Patients were categorized into low (0-1), moderate (2-3), and high-risk (4-9) groups based on their CHA2DS2-VASc scores. One-year mortality was the primary outcome, assessed using survival analysis and multivariate regression.
Results: Higher CHA2DS2-VASc scores ≥4 were associated with older age, increased comorbidities, and elevated inflammatory markers. Survival analysis showed a significant correlation between higher scores and increased mortality risk (HR 5.602, p<0.001), although multivariate analysis did not identify independent predictors of mortality. Laboratory findings varied across risk groups, with significant differences in CRP, uric acid, and calcium levels.
Conclusion: The CHA2DS2-VASc score may serve as a useful tool for mortality risk stratification in haemodialysis patients. However, its predictive value remains uncertain due to study limitations, including a single-centre design and small sample size. Further research with larger cohorts and additional biomarkers is needed to enhance its clinical applicability in CKD populations.
