Effectiveness of exercise based physical therapy management protocols on shoulder function and quality of life after Modified Radical Mastectomy

Abstract
Objectives: To assess the effectiveness of immediate postoperative and home based physical therapy approaches after breast surgery.
Methodology: A convenient sample of 40 patients from 2 major hospitals of Faisalabad was randomly divided into treatment and control groups. Individualized & supervised exercise plan using the Post-op protocol of University of Pittsburgh Medical Center (UPMC) was given to treatment group for four consecutive weeks. The control group after getting discharged received only an exercise brochure containing same exercises. Patients were followed for consecutive 4 weeks and data regarding pain, shoulder range of motion, upper limb disability and quality of life were recorded. SPSS version 20 was used for data analysis.
Results: There was significant improvement (p<0.05) in all four study parameters in both groups. Improvement in pain level was more pronounced in treatment group (p<0.05). Range of motion (ROM), level of upper limb disability and quality of life (QOL) improved more in treatment group except ROM for flexion and abduction which remained same in both groups (p>0.05).
Conclusion: Combination of supervised exercise program during hospital stay and a comprehensive well understood home exercise plan followed by patients undergoing modified radical mastectomy (MRM), resulted in better and rapid recovery in terms of incisional and shoulder joint pain, shoulder ROM, upper limb disability and overall health related quality of life.
