Clinical audit of record keeping for root canal therapy procedures at University Dental Hospital, The University of Lahore, Pakistan

Abstract
Objective: To assess the quality of root canal treatment (RCT) records in the Operative Dentistry Department of our institution to measure compliance with established international guidelines.
Methodology: This cross-sectional audit of 72 clinical records of patients who underwent RCT between November 2024 to January 2025 was conducted. Records were evaluated using a binary scoring system based on ESE guidelines. Data were analyzed using SPSS 21 with parameters including general patient records and endodontic treatment details.
Results: High documentation rates were observed for presenting complaints (94.4%, n=68) and consent (95.8%, n=69), whereas sensibility testing was the least recorded parameter (44.4%, n=32). Among endodontic records, working length and reference point (93.1%, n=67) and canal preparation size (97.2%, n=70) were well documented. However, rubber dam isolation (22.2%, n=16) and canal preparation technique (26.45%, n=19) had low documentation rates.
Conclusion: General record keeping showed strong adherence to key documentation parameters. Deficiencies in endodontic procedural details highlight the need for targeted interventions. Implementing standardized training programs and reinforcing documentation protocols can enhance compliance and improve clinical practice.
