Large water cell parathyroid adenoma associated with primary hyperparathyroidism presented as femur fracture with recurrent renal stone formation

Abstract
Primary hyperparathyroidism is a common cause of hypercalcemia in outpatient practice. The incidence of primary hyperparathyroidism is increasing with age and more common in women than in men. 80-85% of cases of primary hyperparathyroidism are related to single parathyroid adenoma. The majority of parathyroid adenomas contain a mixed cell population with predominant of chief cells. Primary hyperparathyroidism with clear cell adenoma is very rare. There are a few reported cases in the literature with deferent presentation. Skeletal complications of primary hyperparathyroidism are less nowadays and the incidence of renal stones has fallen. Here we report a case of water clear cell adenoma associated with primary hyperparathyroidism presented with severe skeletal and renal manifestation.
