Urinary protein and albumin creatinine ratio for monitoring renal function recovery in women with preeclampsia and eclampsia: early versus late onset

Abstract
Objective: To evaluate the urinary protein and albumin creatinine ratio for monitoring renal function recovery in women with preeclampsia and eclampsia: early (EOP) versus late (LOP) onset.
Methodology: This prospective cohort study at Dr. Soetomo General Hospital enrolled 60 women with preeclampsia, divided into EOP (<34 weeks) and LOP (≥34 weeks) groups. Urinary PCR and albumin creatinine ratio (ACR) levels, as well as blood pressure, were measured before delivery and again 3-6 months postpartum. Data were analysed using appropriate statistical tests.
Results: PCR and ACR decreased significantly postpartum in both groups (p<0.01). The decrease was greater in LOP (ΔPCR=25.4, ΔACR=23.9 mg/mmol) than EOP (ΔPCR=10.77, ΔACR=11.94 mg/mmol; p=0.035 and p=0.014, respectively).
Conclusion: Postpartum PCR and ACR monitoring showed better renal recovery in late-onset than early-onset preeclampsia. These markers help identify patients at risk of persistent renal impairment, highlighting the importance of postpartum screening, especially in early-onset cases.
