Effect of three monthly intra-articular platelet-rich plasma injections on pain, in osteoarthritic knees

Abstract
Objective: To determine the impact of intra-articular Platelet-Rich Plasma (PRP) injections on pain levels in patients with osteoarthritic knees, three months after a three-injection regimen with monthly intervals.
Methodology: The study was conducted at the Department of Orthopedics, Avicenna Medical College and Hospital, Lahore, from October 2024 to March 2025. A total of 70 patients aged 40-80 years with clinically and radiologically confirmed OA (KL Grade 2–4) were enrolled using non-probability consecutive sampling. Patients received three PRP injections at one-month intervals. Pain levels were assessed using the VAS at baseline and after three-months. Adverse events were monitored throughout the study. The treatment was considered effective when pain relief exceeded 50% from baseline after three months.
Results: For Grade 2 OA patients, the VAS scores decreased significantly from 6.69±1.5 at baseline to 3.69±1.4 , for Grade 3 OA, the score decreased from 7.38±1.6 at baseline to 4.75±1.5 and for Grade 4 OA, the score decreased from 7.00±1.4 at baseline to 5.33±1.3 at three months. Mean reduction of 3 points (p<0.001), 2.62 points (p<0.001) and 1.67 points (p<0.05) for Grade 2 OA, grade 3 OA and grade 4 OA, respectively was recorded.
Conclusion: A three-injection PRP regimen significantly reduced knee pain in middle-aged OA patients, with greater efficacy in early-stage disease (Grade 2).
