Abstract
Objective: To identify Non-Alcoholic Fatty Liver Diseases (NAFLD) as the emerging component in Metabolic Syndrome (MS) due to its implications on morbidity and mortality.
Methodology: This observational cross-sectional study with convenient sampling was conducted from June 2023 to May 2024 at Rai Medical College and private consultancies of the contributing authors. Curved Array transducer was used for comparison of hepatic echogenicity with hepatic vessel walls, diaphragm, gall bladder walls, and renal parenchymal echogenicity to decide about the presence of NAFLD and its grading. Data were analysed by using SPSS 25. Chi-Square test of independence was applied to evaluate statistical significance (p<0.001).
Results: Among 278 (53.88%) diabetics, we had 148 (53.23%) males and 130 (46.76%) females. NAFLD was present in 58% of males and 42% of females with diabetes. Among 125 (24.22%) obese patients, 48 (38.4%) were male, and 77 (61.6%) were female. NAFLD was present in 29% males and 22% female obese patients
Conclusion: NAFLD is much more common in MS patients. It is a laboratory diagnosis in asymptomatic persons; the presence of heaviness or pain in the right upper abdomen with raised serum triglycerides and cholesterol, and raised serum ALT are the indications for a closer follow-up.

Key words: Diabetes Mellitus, dyslipidemia, fatty liver, hypertension, metabolic syndrome, nonalcoholic fatty disease of liver, obesity.
